Cost related to surgical procedures or service bundles vary from one surgery center to the next. There you will be able to find general pricing for various procedures and service bundles. Note: The prices mentioned on their website are a non-personalized, general estimate of costs that may be incurred by patients of our facility for anticipated services. You have the right to request a personalized estimate from our facility of the cost that your procedure will incur. To do so, call the number on the contact page of this website.

 

Financial Assistance and Charity Information

Required Information:

Physician Partners of America (PPOA) requires independent information to support the claims of financial hardship including verification of expenses and income. The information submitted with be treated confidentially and will only be reviewed by PPOA administrative staff involved in processing and reviewing information for reduction or waiver of medical expenses.

Time Frame:

After an application and verification information are received, PPOA will consider the overall financial situation of the applicant and then render a decision. All decisions will be made within 10 working days from the time that PPOA receives and reviews all required information. All determinations are pursuant to the hardship guidelines and are discretionary and in the sole determination of PPOA.

Applicants will receive a notification letter outlining whether or not the application has been approved or denied. If an applicant’s request for a reduction or waiver of the charges is denied, PPOA will provide the applicant with written notice of its determination. If the applicant’s financial situation significantly changes. The patient or their designee may reapply.

PPOA Administrative staff will maintain all documentation related to the financial hardship process as confidential. This documentation will include all supporting documentation including the waiver requests and all documents provided in support of the request.

Verification of ongoing qualification for financial hardship may be conducted at any time at PPOA’s discretion or at the applicant’s request.

Financial Hardship Application – How To Begin:

The form can be obtained by visiting any of PPOA’s locations or online by clicking HERE. Forms can also be requested from PPOA’s Patient Experience Department though submission of a written request or U.S. Mail at:

Applicants are required to return the completed form and submit all required documentation to PPOA within 60 days of the date of service.

 

Financial Collection

We accept most insurances and offer different types of payment arrangements just speak to one of our team members prior to your procedure. We collect all co-payments and co-insurances on or before your date of service. Depending on your insurance plan and your deductible balance, you may be asked to pay a portion on or before your date of service, one of our team members will explain this information completely. We ask that you maintain your account balance in good standing. You will receive a statement of your account balance every month for three months until your balance is paid in full. Any other conversations regarding balances owed can be discussed with our dedicated billing department.

 

Health Care Practitioners That Operate Within Our Facility

The following is a list of providers along with their contact information who operate and perform surgical procedures within our facility.

If you anticipate having a procedure at our facility that will be performed by one of the entities listed below, we encourage you to contact them directly for two main reasons:

  1. In order to obtain an estimate of the cost for your specific procedure.
    Note: You will receive separate bills, one from the surgeon, the other from the surgery center. These practitioners will bill you separately for their services, while the surgery center will bill you separately for fees associated with the use of the surgery facility.
  2. To verify if they participate with your insurance company. Keep in mind, there is the possibility that while our surgery facility may accept your insurance, the 3rd party entities below that make use of our facility, may not.
